Abstract
The purpose of this SOP is to provide guidelines for proper, efficiently structured and standardized reporting of notifiable medical conditions (NMC) using the paper based reporting platform in order to facilitate rapid and timeous communication and targeted public health action as stipulated by the National Health Act, 2003 (Act No. 61 of 2003) and the World Health Organisation International Health Regulations 2005 (WHO IHR, 2005).
